Combined Hepatocellular-Cholangiocarcinomav - A case report -

Abstract
Combined hepatocellular-cholargioearcinoma (HCC-CC) is a rare primacy liver cancer with poor prognosis. We experiened a case of combined HCC-CC which had had a surgical treatment. The patient was a 72-year-old man who was admitted for fatigue and epigastric discomfort. There were no abnormal elevations of tumor markers. HBs Ag was positive. On ultasonography, we detected a hyperechoic mass in the right lobe of the liver. Computed tomography showed two found and relatively well defined lesions in the right lobe of the liver: the large one was located at the anterior inferior segment(S5) and measured about 4.0¡¿5.0¡¿5.0§¯ size with low density but high density in arterial physis, the small one was located at the anterior superior segment(S8) and measured about 1.5¡¿2.7¡¿2.1§¯ site with atypical pattern of enhancement. On MRI, both lesions had a low density T1 image and a high density T2 image. Anterior segmentectomy was performed. In the resorted specimen, the large lesion was a well-circumscribed and firm mass with a greenish color that was diagnosed hepatocellular carcinoma, while the small lesion was an ill-demarcated and firm mass with a brownish-to-green color that was diagnosed cholangiocarcinoma. Histologically, each lesion was completely seperated and had a typical pattern of hepatocellular carcinoma and cholangiocarcinoma, respectively; it was diagnosed as a combined hepatocellular-cholangiocarcinoma(Allen¡¯s type¥°).
